The 2025 Form 1095Cs are prepared and available upon request. The Form 1095C provides information about offers of coverage made to full-time employees as well as coverage information for those who enrolled in the Sisters of Charity Health System’s group health plan. Forms will be provided within 30 days of request. This notice will remain available through October 15, 2026.
