There is also a need for better coordination of care with the behavioral health infrastructure in Cuyahoga County, which is vulnerable to a lack of effective communication and collaboration.

Our Services:

  • Screening and assessment of need
  • Individual and group therapy
  • Peer support
  • Case management and care coordination, including linkage to primary care
  • Open door or drop-in availability for those unsure of their needs

OUR MISSION

By offering a more robust, integrated and seamless crisis and recovery continuum, the Crisis & Recovery Services program will provide the right care at the right time, consistent coordination for care transitions and support that addresses social determinants of health.

Increasing Access to Crisis & Recovery Services in Cuyahoga County

The Need

It's no secret that there is tremendous need for support in coping with trauma and violence, especially in underserved neighborhoods. There is also a need for better coordination of care with the behavioral health infrastructure in Cuyahoga County, which is vulnerable to a lack of effective communication and collaboration.

A Coordinated, Community Response

The St. Vincent Charity Health Campus Crisis & Recovery program was designed to help enhance access and address the symptoms of trauma by creating a new coordinated response to crisis in the community and enhancing access to services that support long-term recovery.

The program expands the mental health and addiction crisis continuum of care in Cleveland and Cuyahoga County by partnering with community entities and appropriately addressing all age ranges and acuity levels, particularly lower acuity needs. Our aim is to engage people in their natural settings as often as possible. Visits are community-based, on-site and virtual in nature.

Expanding the Crisis Continuum of Care

The program focuses on crisis outreach, prevention and management efforts to assist individuals who are struggling and in need of support. Our licensed professionals, peers and specially trained staff outreach to local emergency rooms, criminal justice and other critical-time service entities. They provide follow-along support and treatment linkages for people when they are most vulnerable and in need.

Expanding Longer-Term Recovery Supports

Our recovery services professionals offer ongoing, time-unlimited, recovery-oriented services to people who are experiencing serious and persistent mental illness, co-occurring addictions, trauma and medical complexities. Individuals served by this team are at risk of re-hospitalization, more complex and require more treatment than a traditional outpatient clinic can offer.

When to Call or Make a Referral

Here are several referral criteria. In general, a referral is appropriate if an individual:

  1. Is or has recently been in crisis, hospitalized or seen in an emergency setting.
  2. Has trouble functioning and has a serious mental illness.
  3. Is able to receive services as an outpatient and does not need a higher level of care.
  4. Has a need and would like to be linked to care.
  5. Has the ability and willingness to develop a relationship and recovery goals.
  6. Can enhance their mental health and addiction recovery goals through our services and programming.

Referrals may be made by any clinician within the continuum of care, and individuals or their loved ones may self-refer to the program. Please let us know of any barriers to services (transportation, reasonable accommodations) at the time of the referral. If you have referral questions or to help determine if a referral is appropriate, please contact us to further discuss.

The most effective system of care for behavioral health crises combines a coordinated call system (air traffic control), an appropriate mobile crisis response and safe-crisis receiving centers.
