Collaboration between the Sisters of Charity Health System and Catholic Charities Health & Human Services, Inc. integrates services provided by Sisters of Charity Health System sponsored hospitals with community-based Catholic social service organizations and higher education. The partnership encourages all Catholic sponsored providers in the Cleveland Diocese to offer greater continuity of care, especially for the poor and vulnerable.

Key Accomplishments:
• Established a center of excellence for the deaf and hearing impaired.

• Coordinated collaborative effort to provide health care to refugees resettled, through Catholic Charities Migration and Refugee Office.

• Centralized referrals for persons with multiple unmet health and human service needs to appropriate providers throughout the diocese.
